Tăng sản cơ tuyến túi mật với sự tăng sản mỡ dưới thanh mạc: Hình ảnh CT trong hai trường hợp

Adenomyomatosis của túi mật (còn gọi là u cơ tuyến túi mật hoặc tăng sản cơ tuyến túi mật) thường thấy trong các vật phẩm phẫu thuật thông thường. Mỡ dưới thanh mạc có thể tăng lên khi có sự hiện diện của u cơ tuyến túi mật, nhưng lượng mỡ dưới thanh mạc có thể khá khác nhau. Các phát hiện trên hình ảnh CT điển hình và không điển hình trong hai trường hợp tăng sản cơ tuyến túi mật với sự tăng sinh mỡ dưới thanh mạc. Lớp mỡ dày lên bao quanh thành túi mật dày với các túi thoát vị trong lớp cơ và sỏi, hình ảnh CT điển hình  được thấy trong trường hợp một. Lớp mỡ xen kẽ ở ngoại vi của thành túi mật dày mà không có sỏi trong lớp cơ, không điển hình trên CT, được nhìn thấy trong trường hợp thứ hai.

Các phát hiện về bệnh học [1-3] và hình ảnh học [1-5] trong tăng sản cơ tuyến túi mật đã được báo cáo trước đây. Về mặt bệnh lý, lượng mỡ dưới thanh mạc xung quanh túi mật bình thường khá biến đổi [6]. Rõ ràng, mỡ có thể tăng sinh khi có sự hiện diện của bệnh u cơ tuyến và có lẽ cũng trong bệnh lý viêm nhiễm [7]. Tuy nhiên, không có báo cáo nào liên quan đến các kết quả chụp cắt lớp vi tính (CT) trong adenomyomatosis với sự tăng sinh mỡ dưới thanh mạc của túi mật. Chúng tôi báo cáo hai trường hợp adenomyomatosis với sự gia tăng mỡ dưới thanh mạc của túi mật, và trình bày các hình ảnh CT của nó.

Báo cáo các trường hợp

Trường hợp 1

Một phụ nữ 49 tuổi đã tự nhận đau vùng bụng bên phải. Chỉ số xét nghiệm nằm trong giới hạn bình thường. Siêu âm cho thấy tăng âm thành túi mật và sỏi trong lớp cơ túi mật. Kỹ thuật truyền nhỏ giọt chụp hình đường mật cho thấy một vòng thấu xạ bao quanh túi mật chứa đầy tương phản với chất tương phản đầy trong các túi thừa trong lớp cơ.  Chụp cắt lớp vi tính (Hình. 1a) cho thấy một lớp mỡ dày (-32 HU) bao quanh một khối tỷ trọng mô mềm với sỏi trong túi thừa và cổ trướng. Khi phẫu thuật, túi mật mở rộng và chứa sỏi. Thành túi mật dày lên rõ rệt, và độ dày là 20 mm. Mô bệnh học (Hình. 1b) đã thiết lập chẩn đoán của bệnh tăng sản cơ tuyến túi mật với tăng sản lớp mỡ dưới thanh mạc.

Trường hợp 2

Một bệnh nhân nam 34 tuổi đã tự nhận với khó chịu trong bụng. Siêu âm cho thấy thành túi mật tăng âm. Truyền nhỏ giọt chụp hình đường mật cho thấy một vòng thấu xạ mờ nhạt bao quanh túi mật chứa đầy chất tương phản mà không có chất tương phản trong túi thoát vị trong lớp cơ. Chụp cắt lớp vi tính (Hình 2a) cho thấy mỡ xen kẽ ở ngoại vi của thành túi mật dày. Lúc phẫu thuật, thành túi mật dày lên rõ rệt, độ dày khoảng 15 mm. Mô bệnh học (Hình 2b) xác nhận chẩn đoán adenomyomatosis với sự tăng sản xơ mỡ ở vùng dưới thanh mạc.

Thảo luận

Adenomyomatosis túi mật thường thấy trong các vật phẩm phẫu thuật thông thường [2]. Nó cho thấy sự dày lên của lớp cơ và sự tăng sản của biểu mô niêm mạc với sự hình thành của túi thừa trong lớp cơ (xoang Rokitansky-Aschoff). Mỡ dưới thanh mạc có thể tăng sinh khi có sự hiện diện của adenomyomatosis, nhưng lượng mỡ dưới thanh mạc có thể khá thay đổi. Sự tăng sản mỡ dưới thanh mạc liên quan với adenonmyomatosis của túi mật là đáng chú ý trong trường hợp 1, và ít hơn trong trường hợp 2. Trong adenonmyomatosis túi mật, kỹ thuật chụp hình đường mật cho thấy túi mật đầy chất tương phản và túi thoát vị trong lớp cơ [1,2].

Siêu âm cho thấy sự dày lên của thành túi mật và túi thừa trong cơ, được thấy dưới dạng các ổ trống âm hoặc tăng âm trong thành túi mật [1, 3, 4]. Chụp cắt lớp vi tính cho thấy khối tỷ trọng mô mềm trong hố túi mật có giới hạn rõ và một khoảng không chứa u rõ ràng giữa khối và gan, và các túi thừa nhỏ trong cơ và sỏi [4, 5]. Không có báo cáo nào liên quan đến việc xác định sự phát hiện của CT cho sự tăng sinh mỡ dưới thanh mạc liên quan với adenomyomatosis của túi mật. Hình 1 của Boukadoum và cộng sự [5] cho thấy lớp mỡ kế cận các xoang Rokitansky-Aschoff, nhưng họ không đề cập chi tiết đến phát hiện này. Trong trường hợp của chúng tôi, một lớp mỡ rõ rệt bao quanh adenomyomatosis lan tỏa của túi mật đã được nhìn thấy trong trường hợp 1, và mỡ xen kẽ ở ngoại vi của thành túi mật dày rõ rệt đã được nhìn thấy trong trường hợp 2 trên hình ảnh cắt lớp vi tính.

Khi có mô mỡ trong túi mật, lipomatosis của túi mật [8], tăng sinh mỡ dưới thanh mạc kết hợp với u cơ tuyến túi mật, hoặc viêm túi mật mãn tính với mạc nối lớn bao bọc quanh túi mật được bao gồm trong chẩn đoán phân biệt. Một lớp mỡ dày đều có thể được nhìn thấy xung quanh túi mật trong adenomyomatosis với tăng sản lớp mỡ dưới thanh mạc túi mật, cũng như trong lipomatosis của túi mật. Một lớp mỡ mỏng có thể được nhìn thấy xung quanh túi mật, ngoại trừ giường túi mật trong viêm túi mật mãn tính, được bọc bởi mạc nối lớn.


 

Adenomyomatosis of the Gallbladder with Subserosal Fatty Proliferation: CT Findings in Two Cases  

Abstract

Adenomyomatosis of the gallbladder is frequently seen in ordinary operative specimens. Subserosal fat may proliferate in the presence of adenomyomatosis, but the amount of subserosal fat may be quite variable. Typical and atypical computed tomographic (CT) findings in two cases of adenomyomatosis with subserosal fatty proliferation of the gallbladder are presented. The thick fatty layer surrounding the thickened gallbladder wall with intramural diverticula and stones, typical of CT findings, was seen in one case, and fat interspersed in the periphery of the thickened gallbladder wall without intramural stones, atypical of CT findings, was seen in a second case.

Pathological [1-3] and radiological [1-5] findings in adenomyomatosis of the gallbladder have been previously reported. Pathologically, the amount of subserosal fat surrounding the normal gallbladder is quite variable [6]. Apparently, fat may proliferate in the presence of adenomyomatosis and perhaps also in inflammation [7]. However, there are no reports concerning the computed tomographic (CT) findings in adenomyomatosis with subserosal fatty proliferation of the gallbladder. We report two cases of adenomyomatosis with subserosal fatty proliferation of the gallbladder, and present their CT images.

Case Reports

Case 1

A 49-year-old woman was admitted with right abdominal pain. Laboratory data were within normal limits. Ultrasonography showed a hyperechoic wall and intramural stones of the gallbladder. Drip infusion cholangiography showed a thick radiolucent ring surrounding the contrast-filled gallbladder with contrast-filled intramural diverticula. Computed tomography (Fig. la) revealed a thick fatty layer (-32 HU) surrounding a dense soft tissue mass with intramural stones and ascites. At operation, the gallbladder was enlarged and contained intramural stones. Its wall was markedly thickened, and measured 20 mm in thickness. Pathologic examination (Fig. lb) established the diagnosis of adenomyomatosis with subserosal fatty proliferation of the gallbladder.

Case 2

A 34-year-old man was admitted with abdominal discomfort. Ultrasonography showed a hyperechoic wall ofthe gallbladder. Drip infusion cholangiography showed a faint radiolucent ring surrounding the contrast-filled gallbladder without contrast-filled intramural diverticula. Computed tomography (Fig. 2a) revealed fat interspersed in the periphery of the thickened gallbladder wall. At operation, the gallbladder wall was markedly thickened, measuring 15 mm in thickness. Pathologic examination (Fig. 2b) confirmed the diagnosis of adenomyomatosis with fibro-adipose proliferation in the subserosa.

Discussion

Adenomyomatosis of the gallbladder is frequently seen in ordinary operative specimens [2]. It shows microscopic thickening of the muscular layer and proliferation of the mucosal epithelium with formation of intramural diverticula (Rokitansky-Aschoff sinuses). Subserosal fat may proliferate in the presence of adenomyomatosis, but the amount of subserosal fat may be quite variable. The subserosal fatty proliferation associated with adenonmyomatosis of the gallbladder was prominent in case 1, and less dramatic in case 2. In adenomyomatosis of the gallbladder, oral cholecystography shows contrast-filled gallbladder with intramural diverticula [1,2].

Ultrasonography shows thickening of the gallbladder wall and intramural diverticula, seen as anechoic or echogenic foci within the wall [1, 3, 4]. Computed tomography shows dense soft tissue masses in the gallbladder fossa with a well-demarcated and distinct tumor-free space between the mass and the liver, and small intramural diverticula and stones [4, 5]. There are no reports concerning the CT identification of subserosal fatty proliferation associated with adenomyomatosis of the gallbladder. Figure 1 of the Boukadoum et al.’s paper [5] shows fat adjacent to Rokitansky-Aschoff sinuses, but they do not mention this finding in detail. In our cases, a prominent fatty layer surrounding the diffuse adenomyomatosis of the gallbladder was seen in case 1, and fat interspersed in the periphery of the marked thickened gallbladder wall was seen in case 2 on CT scans.

When there is fatty tissue in the gallbladder fossa, lipomatosis of the gallbladder [8], subserosal fatty proliferation associated with adenomyomatosis of the gallbladder, or chronic cholecystitis with the greater omentum wrapped around the gallbladder are included in the differential diagnosis. A uniform, thick fatty layer may be seen surrounding the gallbladder in adenomyomatosis with subserosal fatty proliferation, as well as in lipomatosis of the gallbladder. A thin fatty layer may be seen around the gallbladder, except the liver bed in chronic cholecystitis, which is wrapped with the greater omentum.
